Please note however that use of yahoo email accounts is discouraged as there are chances on non-receipt of email notifications.
The affected workers from reports previously submitted is already reflected and you may click the box of the worker that you wish to be included in the application.
If a worker is not on the list, you may click the “Click Here to Add Workers” tab and complete the required information. However, please note that this will be a new application with a separate CAMP reference number
Upon successful submission of an application, a CAMP Reference Number will appear, kindly take note of this number in tracking status of your application.

Email address is a required field for an individual applying for CAMP Financial Assistance.

Yes. The establishment can submit new application by doing the same process. The application will be considered as a separate application and will be provided with a separate tracking number.
Errors may be experienced due to slow internet/lagging internet connection, or this may be due to high traffic of submission of applications for CAMP. Kindly click OK, and try submitting again.
If reason for denial was due to submission of wrong document/s, re-application may be done through the System by attaching the correct documents.
Check that all required fields have been properly filled up or selected from the drop-down selections. The required attachments should be PDF, JPEG, or PNG format and should not exceed 10MB.

Registration of establishments in the DOLE Establishment Report System should be for each branch or office because the reports are usually submitted to the DOLE Regional/Provincial/Field Office
where the company is registered.
For concessioners/tenants of a department or grocery stores, the registration and reporting will be based on the location where they are deployed.

In registering a branch store/establishment, the establishment name should be based on the business permit.
The brand name (franchise name) should be indicated once the franchise is selected under the “Establishment Classification”.

You may log in to your establishment’s account at https://reports.dole.gov.ph/ and click “View Submission”. All submitted reports can be viewed and downloaded.

May we suggest inputting entries in batches and click the “accept” button to submit the report. Please wait for the pop-up window confirming that the submission was
successful before starting the next batch. To ensure that the submission was successful as well, you may click the “View Submission” button to view submitted reports.
